For your convenience and on some time sensitive notifications we have available some of the forms for download in PDF format with highlighted fillable fields. Unless otherwise stated the following conditions are encoded in the file, and instructions apply to all documents. If you need the latest free Adobe Acrobat Reader you can click on this icon to go to their site. This will work an MAC or PC.
1.) After checkout click on the "here" underscore and you will be in another page of your WEB Browser. There you are, In Description click on download and you will be prompted where to save your file.
2.) Download to your hard drive (make a folder or use My Documents of going to a PC or use your download folder on MAC).
3.) Open the document in Reader and when instructed go to File>Save. On saving the document for the first time, this will activate the fillable fields and they will be highlighted in blue.
4.) At this time you can fill the fields as necessary and print the completed form. NOTE since many of our forms are on multi-part carbonless paper, you will have to print your desired number of copies on your paper. You can save this data, goto File>Save to save your changes.
5.) These forms have an expiration and not sold for permanent use. You can print 7 copies, after that the word "Expired" is printed accross the page. After 5 days the word "Expired" is shown across the screen, which ever comes first. In both cases the fillable fields are disabled.
