This book is for all Contractors who perform Renovation, Repairs, or Painting on homes or buildings built prior to 1978 must provide Owners, Tenants, and Child-Care facilities with a copy of EPA's Lead Hazard Information Pamphlet. Renovate Right has important lead hazard information.

This form is used as a signed confirmation of deliverence of the Renovate Right Booklet. Contractors must document compliance of this EPA requirement with a signed Pre-Renovation form. This is a 2 part carbonless form. Revision effective April, 2010.
